BL4 Pain & Terror, Twin Boss Fight Guide

Pain and Terror are a duo boss encounter on Nekrotafeyo and tbh the first time I fought them I had absolutely no clue what was going on because two bosses sharing a health pool with completely different attack patterns is just chaos and I died like 5 times before I even figured out which one was Pain and which one was Terror and honestly who names their bosses like this anyway but I mean it fits because that's exactly what they cause you, pain and terror. They share a health pool but have separate mechanics and Pain charges at you while Terror hangs back and debuffs you and you have to manage both simultaneously while prioritizing whichever one has the active mechanic and it's honestly one of the more unique boss encounters in the game even if it can get kind of frustrating ngl especially when they sync up their attacks perfectly to delete you from existence.

Farming Strategy

The first thing I learned was to stop standing in the middle of the arena like an idiot

Pull both bosses together and use weapons with chain effects like the Reflux or Flipper to damage both simultaneously because the shared health pool means any damage to either boss counts and you can basically treat them as one big target if you keep them grouped up and it makes the fight so much faster. Save your action skills for when the Terrify debuff is active to survive through it and burst them down while debuffed because the 50% damage reduction really hurts your output and you need the extra oomph from action skills to compensate. Kill within 2 minutes for optimal farm speed and if you're taking longer than that you probably need better chain damage weapons or a stronger build and I've been there where the fight dragged on for like 5 minutes and it was honestly miserable. Oh and one more thing, the loot drops from whichever boss dies second so position yourself near the center of the arena when the health bar is low so you don't have to run across the whole arena to grab your drops and waste time, it's a small optimization but it adds up when you're farming for a perfect Baby Maker++ or Stop-Gap and trust me you're gonna want that Stop-Gap it's one of the best defensive shields in the game imo.

Detailed Fight Strategy

The Pain & Terror fight is unique because you're dealing with two bosses that share a health pool but have completely different behaviors and honestly it took me way too long to figure out the rhythm of this fight

Pain is the aggressive one and he charges at you in a straight line and destroys any cover in his path and Terror hangs back and casts debuffs on you from a distance and the arena is this ruined amphitheater on Nekrotafeyo with broken pillars and elevated platforms around the edges and it feels like you're fighting in a collapsed colosseum which is pretty cool atmosphere wise even if the fight itself can be super annoying. So here's how you want to approach this, at the start immediately run to the side platforms and don't stand in the center of the arena because that's basically asking to get charged by Pain and debuffed by Terror at the same time and I've made that mistake so many times it's honestly embarrassing. Pain's charge has a really clear audio cue and you'll hear this mechanical grinding sound about a second before he charges so dodge sideways never backward because if you dodge backward he tracks you and hits anyway and I learned this by dying to charges over and over until it finally clicked. The pillars in the arena block his charge but they break after one hit so rotate to new pillars after each charge and don't get attached to any single piece of cover because it's gonna be gone in about 3 seconds and then you're standing in the open wondering what happened.

Terror's debuff is honestly the most dangerous part of this fight and I cannot stress this enough

When the Terrify debuff is on you your damage drops by 50% and you move slower and you'll see a purple haze around your screen when it's active and that purple haze is basically a death sentence if you don't react to it immediately. Break line of sight with Terror to remove it and the side platforms have these broken walls that work perfectly for this and the moment you see purple just duck behind a wall and wait for it to clear because trying to fight through the debuff is suicide and I've tested this theory extensively by dying a lot. The Synergy Attack is the one-shot mechanic and if both bosses hit you at the same time you're dead regardless of your shield and this happens most often when you're standing between them so the golden rule of this fight is never have Pain and Terror on opposite sides of you and keep them both in front of you or better yet kite them so they're close together. Chain damage weapons like the Reflux absolutely shine here because the bosses naturally group up and every bullet bounces between them and you're effectively doing double damage and it's satisfying in a way that few other boss fights manage to deliver imo.

For the Summon Guardians move honestly just ignore them

They don't do much damage and they're tanky enough that killing them wastes time and focus all damage on the bosses because the shared health pool means any damage to either boss counts and you should prioritize whichever one is easier to hit at any given moment. The fight should take about 2 minutes with decent gear and if it's taking longer you probably need to upgrade your weapons or rethink your approach because a long Pain & Terror fight is a dead Pain & Terror fight and you'll run out of cover and get worn down by chip damage and the debuff cycling and eventually they'll sync up a charge and a debuff at the same time and delete you and it's just not worth dragging it out, kill them fast or they kill you that's pretty much the deal.

Best Vault Hunters for This Boss

Amara is the queen of this fight and it's not even close and I say this as someone who mains Moze

Amara's Ties That Bind is tailor-made for dual boss encounters and you link both bosses together and all your damage gets shared and amplified and use a Reflux or Flipper for the chain damage and you can basically treat them as one giant health bar and it's honestly beautiful to watch. Her Phasegrasp can temporarily lock down Pain when he's charging which is hilarious and also very effective and if you're running a Ties That Bind build with a Phasezerker class mod this fight becomes almost trivially easy and you'll wonder why you ever struggled with it in the first place. Moze is also excellent because Iron Bear can tank Pain's charges without flinching and the splash damage from Short Fuse hits both bosses at once and a Blast Master Moze with a Flipper just holds down the trigger and everything dies and it's very satisfying in a brute force kind of way. Zane does well because his Digi-Clone gives you a second target for both bosses to split their attention between and the clone draws aggro and gives you breathing room and it's surprisingly effective even if it's not as fast as Amara or Moze. Fl4k is probably the weakest here because his single target focus means you're only damaging one boss at a time and his pet often dies to the Synergy Attack and it's just sad honestly but you can make it work with the right build and a lot of patience and stuff like that. Anyway ranking is Amara then Moze then Zane then Fl4k and honestly if you have an Amara with Ties That Bind you should just use her because the fight becomes almost trivial and you'll save yourself a lot of frustration tbh.

Mayhem Mode Differences

At M4+ the Terrify debuff becomes way more punishing and the damage reduction stays at 50% but the movement slow gets much worse like you're basically walking through molasses and you absolutely need to break line of sight with Terror immediately or you're a sitting duck for Pain's charge and I cannot count how many times I've been caught by a charge while trudging around at half speed feeling like I was underwater. At M6+ Pain's charge starts one-shotting through most shields if you're debuffed so the Terrify debuff goes from annoying to lethal and you really can't afford to let it stay on you for more than a second or two and it changes the whole flow of the fight because debuff management becomes more important than DPS.

Mayhem 10 Pain & Terror is one of the harder M10 duo fights in the game

Both bosses get massive health scaling and the shared pool means you're effectively fighting a boss with double HP and you need a really optimized build with chain damage or splash damage to get through it efficiently and I've spent hours trying different builds on this fight and trust me some setups just do not work no matter how good they are elsewhere. Modifier wise Galaxy Brain is decent because both bosses have critical spots and Speed Demon helps dodge charges which is always nice but avoid Dazed and Infused at all costs because random elements mess up your damage so badly on this fight. Buddy System makes the Guardian summons way worse and you'll have like 10 adds running around and it's chaos and not the fun kind. Healy Avenger is actually incredible here because the healing drone offsets the constant chip damage you take from being in a dual boss arena and it's one of the few fights where I actively look for that modifier. Pool Party or floor hazards are bad because the arena already has limited safe space and Post Mortem is death so reroll it and honestly M10 modifier RNG can make or break this fight and sometimes you spend more time rerolling than fighting and that's just Mayhem 10 life you get the idea.

Speed Farming Tips

This isn't the fastest farm in the game but you can optimize it and I've put in the hours to figure out the shortcuts

The spawn point at Tazendeer Ruins is about a 40 second run to the arena and the key to speed is grouping the bosses together and using chain damage weapons because a Reflux with a good anointment can kill both bosses in about a minute since every bullet bounces between them and it's honestly the most efficient way to farm them. Save your action skill for when both bosses are grouped up and Amara's Phasegrasp into Ties That Bind into a full Reflux mag dump is basically the speed kill script and it's so satisfying when it all comes together and you just watch both health bars melt simultaneously. For Moze hop into Iron Bear immediately and use explosive miniguns and the splash hits both targets and you can pretty much ignore the mechanics for the first 20 seconds while you burn through a huge chunk of their health. Don't bother with the Guardian adds at all because they're a time waste and they don't drop anything good and just ignore them and focus on the bosses. One trick that saves a surprising amount of time is that the loot drops from whichever boss dies second so position yourself near the center of the arena when the health bar is low and you won't have to run across the whole arena to grab your drops and it saves maybe 5 seconds per run but that adds up to minutes over a farming session. But the dedicated drops from this fight are actually really good and Baby Maker++ and Night Hawkin are both top tier weapons and the Stop-Gap is one of the best defensive shields in the game and honestly the loot pool makes the farming worth it even if the fight can be annoying at higher Mayhem levels and stuff like that, it's a solid farm imo.

Common Death Causes

Getting hit by the Synergy Attack

This is the number one killer and it happens when you're standing between both bosses and they both attack at the same time and the damage is coded as a one-shot mechanic and there's no surviving it no matter how good your gear is. Always keep both bosses in your field of view and never let them flank you and I cannot tell you how many times I've been killed by this because I got greedy and tried to DPS while standing in the middle of the arena and both bosses just turned and deleted me in a single frame. The second biggest killer is Pain's charge while you're Terrify debuffed and you're moving slow and can't dodge effectively and the charge does enough damage to down you through a full shield so if you get debuffed stop attacking and break line of sight with Terror immediately and I mean drop everything and run to cover because fighting through the debuff is a death wish.

The Guardian summons seem harmless but they're not

They block your shots and body block your movement and I've died more times than I'd like to admit because a Guardian stepped in front of me while I was dodging a charge and I got stuck on its hitbox and then Pain hit me and I was dead. If they're in your way kill one or two but don't make a project out of clearing them all because that wastes time and they'll just respawn anyway. And the arena itself is a hazard and some of those broken pillars have weird collision and you can get stuck on the geometry while dodging and that's happened to me more times than I can count and it's infuriating every single time. Stick to the outer platforms where the terrain is more predictable and avoid the center of the arena as much as possible. Last thing and I mean this seriously, don't use melee builds here because Pain's melee slam punishes close range and Terror can debuff you before you get into melee range and you'll just be running around trying to get close while constantly getting pushed back and debuffed and it's miserable. Mid-range weapons with chain damage or splash are the way to go and anything else is just making the fight harder than it needs to be and honestly I tried melee once and it was the worst 10 minutes of my Borderlands career, just don't do it.
